Usually any loss of sychronisation between hob and job rotation is caused by backlash in differential and/or index gear trains apart from that which may arise from from loose keys / oversize keyways of gears or interconnecting shafts. Also, any backlash in axial leadscrew and boxnut could cause this to happen.
